Records Request

If LYFD responded to a incident that you were involved in and you would like to receive a copy of the report, please complete the form below.

Please allow 7 business days for the report to be filed in our system.

You will be required to fill out a form for our files. This is merely to maintain a chain of custody for the report. This form can be sent by fax or e-mail.

If any incident has been investigated by the Harris County Fire Marshals Office, requests may take longer until the report is released by them.

LYFD does not record and/or keep medical reports on file, please refer to the EMS services that respond in our area for medical records:

This is only required if you are requesting the record to be faxed to you.
Please include the time as well, if known.
To help us locate the record, please be a specific as possible.
House fire, vehicle fire, vehicle accident, etc...
Property owner, insurance agent, etc...
Please provide the nature of your request and any other details that may help us locate the requested incident.
